Constellation Unity Foundation connects older adults, children, and low-income families across Central Florida to the mental health care and community resources they need. We remove the barriers that keep people from starting care and the barriers that keep them from finishing it, and we work to show that mental health means more than a diagnosis. It means living with dignity and stability too.

A dedicated case manager stays with each person from their first appointment through the full course of treatment, ensuring follow-through.
Companion Services
Trained companions offer support and connection, so no one has to face treatment alone. We provide the companionship they need to heal.
What We Do
Since 2024, Constellation Unity Foundation has helped over 200 older adults access in home assistance, including referrals for home health aide services, help with utility and rent payments, home furnishing, and grocery shopping. We also provide transportation to medical appointments, the grocery store, and the pharmacy, so people can live independently and stay connected to their community.
